Most bad breath starts in your mouth.

Cleveland Clinic names the top of that list directly: the main cause of halitosis is poor oral hygiene.

Bacteria on the back of the tongue. Johns Hopkins Medicine says certain bacteria on the back of the tongue interact with amino acids in foods and make smelly sulfur compounds. This is the most commonly missed cause because a toothbrush aimed at teeth never gets there.

Food and plaque left between the teeth. Mayo Clinic says the breakdown of food particles in and around your teeth leads to more bacteria and a foul odour. If you do not brush and floss daily, plaque forms and stays.

Dry mouth. Saliva rinses your mouth continuously. Cleveland Clinic says not having enough leads to halitosis, and that many medicines and health conditions cause dry mouth. Smoking dries it too.

Gum disease. Plaque on teeth and tongue causes gingivitis, the early and reversible stage. Cleveland Clinic says untreated gingivitis leads to periodontitis, which involves bone loss. Mayo names gum disease as a source of bad breath in its own right.

Tonsil stones. Cleveland Clinic describes food getting stuck in the tonsils and hardening into bad smelling calcium deposits at the back of the throat. They are small, easy to miss, and smell far worse than they look.

Infections in the nose, throat or lungs. Cleveland Clinic says infections here can lead to foul smelling fluid at the back of the throat. This is the cause that needs a doctor rather than a better toothbrush.

No. 1

The main cause of halitosis is poor oral hygiene, which allows harmful bacteria to invade the mouth and cause bad smells.

Source: Cleveland Clinic

Five of those six live in the mouth and throat. Bad breath with bleeding gums, loose teeth, a persistent bad taste, or a fever and sore throat needs a dentist or doctor rather than a stronger rinse.

Frequently Asked Questions
<details>
Why does my breath smell even though I brush twice a day?

Because brushing your teeth is not the same as cleaning your mouth. Johns Hopkins says the bacteria that produce the smelly sulfur compounds sit on the back of your tongue, and a toothbrush aimed at teeth never reaches them. Add the tongue and daily flossing before you assume something deeper is wrong.
</details><details>
Is bad breath a sign of a stomach problem?

Usually no. Mayo Clinic states that most bad breath starts in your mouth, and its causes list is dominated by food, tobacco, incomplete cleaning and gum disease. Other conditions can contribute, but the mouth is where to look first and where most cases are solved.
</details><details>
Does mouthwash actually work?

For the smell, briefly. For the cause, not really. Cleveland Clinic says some mouthwashes only mask odours and Johns Hopkins says they only mask the odour for a short time. If you use one, Cleveland suggests an alcohol free antibacterial rinse, since alcohol dries the mouth and dry mouth is itself a cause.
</details><details>
How do I know if it is my gums?

Bleeding when you brush is the everyday signal, and gum disease often has no pain at all in its early stage. Cleveland Clinic describes gingivitis as the early form that plaque causes, progressing to periodontitis with bone loss if untreated. A dentist can see it in minutes, which is the argument for the checkup.
</details><details>
Can my medications be causing this?

Quite possibly, through dry mouth. Cleveland Clinic says lots of medicines and conditions cause dry mouth, and that not having enough saliva leads to halitosis. Ask your pharmacist to look down your list for the dry mouth offenders. It is a two minute conversation and it costs nothing.
</details><details>
What are tonsil stones and would I know if I had them?

They are small deposits that form when food gets stuck in the tonsils and hardens, and Cleveland Clinic names them among the causes of bad breath. Many people never see them, because they sit at the back of the throat. If your routine is good and the smell persists, ask to have your throat looked at.
</details>

Wondering where you stand overall? Take the free 5-minute Healthspan Snapshot and see your strongest and weakest of six dimensions.
