<p>About half of men show visible hair loss by 50. Two FDA-approved drugs treat it. They work in completely different ways, carry different side effects, and cost different amounts. Most men who try treatment quit within a year because they did not understand what to expect from either drug going in.</p>
<p>Here is the head-to-head between finasteride and minoxidil: how each works, what the data shows on effectiveness, what side effects to weigh, and which order most dermatologists recommend if you are starting fresh.</p>
<h3>How Finasteride Works (And What It Actually Does):</h3>
<p>Finasteride is an oral pill (1 milligram daily for hair loss) that blocks the enzyme 5-alpha reductase. That enzyme converts testosterone into dihydrotestosterone (DHT), the hormone responsible for shrinking hair follicles in male pattern baldness.</p>
<p>Less DHT in the scalp means follicles stop miniaturizing, and many follicles that were dormant return to active growth. Trials show finasteride halts hair loss progression in 80 to 90 percent of men and produces visible regrowth in roughly two-thirds (<a href="https://www.health.harvard.edu/skin-and-hair-health/treating-hair-loss-in-men-what-works" target="_blank" rel="noopener">Harvard Health, 2024</a>).</p>

<p>Topical minoxidil (2 or 5 percent solution or foam, applied twice daily) is available over the counter and shows visible improvement in 30 to 40 percent of users. Oral minoxidil (low-dose 1 to 5 milligrams daily, prescribed off-label) has gained popularity in the last 5 years as more effective with less daily hassle, though it carries small cardiovascular risks at higher doses (<a href="https://www.mayoclinic.org/drugs-supplements/finasteride-oral-route/description/drg-20063819" target="_blank" rel="noopener">Mayo Clinic, 2024</a>).</p>
<h3>Head-to-Head: Which Works Better:</h3>
<p>In direct trials, finasteride alone outperforms topical minoxidil alone on most measurements (hair count, hair thickness, patient satisfaction at 12 months). About 65 to 75 percent of finasteride users see meaningful regrowth versus about 30 to 40 percent on topical minoxidil.</p>
<p>The combination outperforms either drug alone. A 2025 retrospective review of combined oral minoxidil plus finasteride showed stable or improved hair in 92 percent of users at 12 months. <h3>The Side Effect Profile You Should Know About:</h3>
<p>Finasteride side effects affect roughly 2 to 5 percent of users in randomized trials: lower libido, erectile dysfunction, and reduced ejaculate volume. Most reverse within weeks of stopping the drug. A small subset of men report persistent sexual or mood effects after stopping, sometimes called post-finasteride syndrome. The condition is recognized but the exact prevalence is contested.</p>
<p>Topical minoxidil's most common side effects are scalp irritation, dandruff, and unwanted facial hair growth (when the solution drips off the scalp). Oral minoxidil can cause fluid retention, mild swelling in legs, racing heart, and dizziness. These are dose-dependent and uncommon at the low doses used for hair loss.</p>
