<p>Hair loss after 50 is one of those subjects where the gap between marketing and evidence is enormous. The shelves are crowded with shampoos, vitamins, scalp tonics, oils, and devices, all promising thicker hair. The published evidence supports a much shorter list. Here are the four interventions with real data behind them and three that are pulling money out of your pocket without changing your outcome.</p>
<p>This article covers age-related pattern hair loss in both men and women. If you have sudden hair loss in patches, hair coming out in clumps, scalp pain or scaling, or thinning brows, that is a different problem (alopecia areata, telogen effluvium, autoimmune disease, thyroid disorder) and warrants a dermatology visit before any of this applies.</p>
<h3>Treatment 1 That Works: Topical Minoxidil:</h3>
<p>Minoxidil is the over-the-counter foam or liquid sold as Rogaine and a hundred generics. Per <a href="https://www.health.harvard.edu/skin-and-hair-health/treating-hair-loss-in-men-what-works" target="_blank" rel="noopener">Harvard Health, 2024</a>, it is FDA-approved for both men and women, decades old, well-tolerated, and produces measurable hair density improvement in roughly 30 to 60 percent of users.</p>
<p>The catch is consistency. Minoxidil works only as long as you use it. Stopping returns the scalp to its prior shedding pattern within a few months. Expect 4 to 6 months before visible change, and plan on continuous use indefinitely. Twice-daily application is standard; once-daily may work but with weaker data.</p>
<h3>Treatment 2 That Works: Oral Finasteride (Men):</h3>
<p>Finasteride (Propecia) is a prescription oral pill that blocks the conversion of testosterone to dihydrotestosterone (DHT), the hormone driving male-pattern baldness. According to a 2025 review cited by <a href="https://www.health.harvard.edu/skin-and-hair-health/treating-hair-loss-in-men-what-works" target="_blank" rel="noopener">Harvard Health, 2024</a>, oral finasteride is slightly more effective than topical minoxidil based on hair density change at 6 months.</p>
<p>Side effects matter. A small percentage of men experience reduced sex drive, erectile dysfunction, ejaculation issues, breast tenderness, or mood changes. Some side effects can persist after stopping the drug. Pregnant women must not handle broken pills due to fetal-development risk. Talk through the trade-off honestly with a clinician.</p>
<h3>Treatment 3 That Works: Topical or Oral Treatments for Female Pattern Hair Loss:</h3>

Spironolactone (an oral diuretic with anti-androgen effects) is another option for women whose hair loss has a hormonal driver.</p>
<p>Hormone replacement therapy in postmenopausal women has not been shown to reverse hair loss directly, but it may improve overall hair density in a subset of women whose loss tracked with hormonal change.</p>
<h3>Treatment 4 That Works: Treating the Underlying Cause:</h3>
<p>Iron deficiency, thyroid dysfunction, low vitamin D, and severe protein deficiency can all cause hair loss that mimics pattern thinning. Per Harvard, a basic workup including ferritin, TSH, and vitamin D is reasonable for anyone with noticeable thinning, especially women. Correcting an underlying deficiency can produce real regrowth even without other intervention. This is the highest-leverage step that gets skipped most often.</p>
<h3>What's Wasting Your Money: Biotin Supplements:</h3>
<p>Biotin (vitamin B7) is the marquee ingredient in nearly every "hair, skin, nails" supplement. Per <a href="https://www.health.harvard.edu/diseases-and-conditions/vitamins-minerals-and-hair-loss-is-there-a-connection" target="_blank" rel="noopener">Harvard Health, 2024</a>, biotin supplementation only helps in the rare case of true biotin deficiency, which is uncommon in adults eating a normal diet. The trials do not support biotin as a hair-growth intervention in non-deficient adults. Worse, high-dose biotin can interfere with thyroid and cardiac lab tests, producing false readings.</p>
<h3>What's Wasting Your Money: Most Hair Growth Shampoos and Tonics:</h3>
<p>The shampoo category is the biggest source of marketing-versus-evidence mismatch in hair loss. Shampoos sit on the scalp for less than a minute and contain ingredients that, even if active, have nowhere near the contact time needed to do anything. The exception is ketoconazole shampoo, which has some evidence for inflammation-mediated thinning. Most other "growth" shampoos are conditioners with confident packaging.</p>
<h3>What's Wasting Your Money: At-Home Laser Caps and Brushes:</h3>
<p>Low-level laser therapy (LLLT) caps and combs have FDA clearance (which is not the same as FDA approval and is much easier to obtain). Trial data is mixed, with some studies showing modest density improvement and others showing no effect. Compared to the cost (often 500 to 1,500 for a quality device), the evidence does not support purchase as a first-line investment. If you have already tried minoxidil and finasteride and want to add something, this is a discussion to have with a dermatologist, not a TikTok ad.</p>
