<p>You probably heard the headlines in 2023. The first drug to actually slow Alzheimer's. A breakthrough after 20 years of failed trials. Hope, finally, for the families who have spent decades watching a loved one disappear in slow motion. The drug is called lecanemab, sold under the brand name Leqembi, and the headlines were not wrong. But they were not the whole story either.</p>
<p>What got lost in the celebration is what the drug actually does, who it actually helps, and what the trade-offs look like in real life. The honest answer is more useful than the headline. If you have a parent, a spouse, or a friend asking whether lecanemab is worth considering, here is what the published Tier 1 evidence shows.</p>
<h3>What Lecanemab Actually Does</h3>
<p>Lecanemab is an intravenous monoclonal antibody. It binds to beta-amyloid, a sticky protein that builds up between brain cells decades before symptoms appear and is one of the defining features of Alzheimer's disease. Once bound, the immune system clears the amyloid out.</p>
<p>In the 18-month Clarity AD trial of 1,795 participants, lecanemab slowed the rate of cognitive and functional decline by 27% compared to placebo on the primary endpoint, the Clinical Dementia Rating Sum of Boxes score (<a href="https://www.nia.nih.gov/health/alzheimers-treatment/how-alzheimers-disease-treated" target="_blank" rel="noopener">NIA, 2024</a>). On secondary measures of daily function, the slowdown ranged from 24% to 37%.</p>
<p>A 27% slowdown is not a cure. It is not a reversal. It is a meaningful delay in the rate at which a person loses themselves. For a family in the early stage of this disease, that delay can be the difference between attending a granddaughter's wedding with recognition and not.</p>
<h3>Who Qualifies (And Who Does Not)</h3>
<p>Lecanemab was studied and approved only for adults with mild cognitive impairment or mild dementia stage of Alzheimer's, confirmed by a PET scan or cerebrospinal fluid test showing amyloid pathology. People with moderate or advanced Alzheimer's were not in the trial and do not qualify for treatment.</p>
<p>The eligibility window is narrower than most people realize. By the time many families seek a diagnosis, the disease has progressed past the mild stage. That is why earlier evaluation matters so much when lecanemab is on the table.</p>
<p>The FDA granted lecanemab traditional approval in July 2023, converting it from the accelerated approval pathway that had cleared it six months earlier (<a href="https://www.nia.nih.gov/news/fda-converts-novel-alzheimers-disease-treatment-traditional-approval" target="_blank" rel="noopener">NIA, 2023</a>). Medicare now covers it for patients who meet the early-stage criteria.</p>
<h3>The Side Effect Most People Do Not Hear About</h3>
<p>The most important risk is called amyloid-related imaging abnormalities, or ARIA. As the drug clears amyloid plaques, the surrounding blood vessels can temporarily leak, causing brain swelling (ARIA-E) or small bleeds (ARIA-H). In the Clarity AD trial, 21% of treated patients showed some form of ARIA on MRI, compared to about 9% in the placebo arm.</p>
<p>Most ARIA cases are mild and resolve without symptoms. But about 3% of patients experienced symptomatic ARIA, and rare deaths have been reported, particularly in patients taking blood thinners or carrying two copies of the APOE4 gene variant (<a href="https://www.mayoclinic.org/diseases-conditions/alzheimers-disease/diagnosis-treatment/drc-20350453" target="_blank" rel="noopener">Mayo Clinic, 2024</a>). MRI monitoring during the first months of treatment is now standard.</p>
<p>This is the part that gets minimized in the marketing. The drug works, and the risks are real, and a thoughtful conversation with a specialist is the only way to weigh them for a specific person.</p>
<h3>What This Means for Alzheimer's Research</h3>
<p>Lecanemab is proof of a principle that took two decades and dozens of failed trials to establish: removing amyloid from the brain slows the disease. A second drug, donanemab, has since shown similar effects with a different dosing schedule. More antibody therapies are in late-stage trials.</p>
