<p>NMN and NR have been marketed as the supplements that "reverse cellular aging." The pitch is appealing: NAD+ declines with age, NAD+ runs your mitochondria, raise NAD+ and you turn the clock back. The pitch is also several jumps ahead of the actual data.</p>
<p>The newer studies have not vindicated the most aggressive claims. They have softened them. Here is what the trials actually show, what they do not, and how a careful consumer might think about it.</p>
<h3>What NMN and NR Actually Are</h3>
<p><strong>Two NAD+ Precursors:</strong> Nicotinamide mononucleotide (NMN) and nicotinamide riboside (NR) are both vitamin B3 derivatives the body converts to NAD+, a coenzyme cells use for energy metabolism and DNA repair. Older adults have lower tissue NAD+ than younger adults, and the longevity hypothesis is that raising it back should turn back at least some functional aging (<a href="https://www.nia.nih.gov/news/nicotinamide-supplement-aging-mice-shows-some-health-improvements-not-increased-longevity" target="_blank" rel="noopener">NIH NIA, 2024</a>).</p>
<p>The hypothesis is reasonable. The clinical question is whether oral supplementation actually delivers, and at what magnitude. That is the question the trials are answering, slowly.</p>
<h3>What the Animal Studies Showed</h3>
<p><strong>Better Metabolism, Not Longer Lives:</strong> Aging mice fed NAM (a closely related precursor) over more than a year showed better glucose handling, less fatty liver, and better physical performance than expected for their age. Important detail: lifespan was not extended. The intervention improved how the mice aged without giving them more years.</p>
<p>This pattern, "healthspan" improvement without lifespan gain, is the more honest framing for what these precursors might offer. It is not nothing. It is also not the headline a supplement label wants to put on a bottle.</p>
<h3>What the Human Trials Have Found</h3>
<p><strong>The Numbers Are Real, the Effects Are Modest:</strong> NR has been tested in over two dozen published human trials, mostly in adults, at doses up to 1,000 to 2,000 mg per day for up to several months. The clearest finding is that NR raises blood and tissue NAD+ levels reliably and safely. Downstream effects on the things people actually care about (muscle insulin sensitivity, blood pressure, cognition, walking distance in peripheral artery disease) have been small, mixed, and often missed primary endpoints.</p>
<p>A 2022 trial in older women showed a modest but real improvement in muscle insulin sensitivity with NMN. A 2024 NICE trial in peripheral artery disease showed a clinically meaningful walking improvement among compliant users. Trials in cognitive impairment and long-COVID have shown more equivocal results.</p>
<h3>Why the Marketing Outruns the Data</h3>
<p><strong>Where the Hype Comes From:</strong> Most of the dramatic "reverse aging" claims trace back to animal studies, in vitro work, or biomarker shifts (blood NAD+, sirtuin activation) rather than the outcomes patients live with. When research news bulletins are translated to consumer marketing, "improved one biomarker in mice" becomes "reverses cellular aging in humans" in a way that does not survive scrutiny (<a href="https://www.nccih.nih.gov/health/providers/digest/myth-busting-popular-natural-products-marketed-for-disease-prevention-and-wellness-science" target="_blank" rel="noopener">NIH NCCIH, 2024</a>).</p>
<p>Add a $40 to $80 monthly price tag and a confident podcast appearance and you have a product that sells well regardless of what the trials say.</p>
<h3>Safety, Quality, and How to Think About Trying It</h3>
<p><strong>Short-Term Safety Looks Okay; Long-Term Is Unknown:</strong> In trials lasting weeks to several months, both NR and NMN have looked well-tolerated at typical doses. There are some theoretical concerns about whether sustained NAD+ elevation could accelerate growth of subclinical cancers, since NAD+ also fuels tumor metabolism. The data is too thin to call that a real risk, but it is the most cited reason NIH-funded researchers urge caution rather than enthusiasm (<a href="https://www.nccih.nih.gov/health/tips/tips-natural-products-used-for-common-agingrelated-conditions" target="_blank" rel="noopener">NIH NCCIH, 2024</a>).</p>
<p>If you decide to try one, NR has the deeper trial record. Stick to USP-verified or NSF-certified brands, and skip the "longevity stacks" combining NMN with five other unverified compounds. Watch for clinical-trial enrollment opportunities; the data is far more useful inside a structured study than from a personal experiment.</p>
<h3>The Boring Levers Still Outperform</h3>
<p><strong>What Actually Has Lifespan Evidence:</strong> Regular exercise (especially zone 2 cardio plus strength training), a Mediterranean-style diet, 7 to 8 hours of sleep, and not smoking remain the only interventions with population-level lifespan and healthspan data. NMN and NR may have a future role. They are not a substitute for the basics.</p>
