Dermatophytes cause 90 percent of cases. <span class="acv-s">Cleveland Clinic</span></div><div class="acv-item"><b>The drugstore cream cannot work.</b> The American Academy of Dermatology is explicit that over-the-counter terbinafine cream treats skin, not nails, because it cannot penetrate the nail. <span class="acv-s">AAD</span></div><div class="acv-item"><b>No laser is approved to cure it.</b> The FDA has approved laser treatment only to improve the look of a nail after the fungal infection has cleared. It has not approved any laser to treat the infection. <span class="acv-s">AAD</span></div></div><div class="acv-body"><p class="acv-dropcap">Toenail fungus can be treated, but almost nothing sold over the counter can treat it. The medicine has to get through the nail, and creams do not.</p><p>A mold called a dermatophyte feeds on keratin, the protein that makes nails hard. It settles under the nail plate, where products made for skin cannot follow.</p><p>Here are the six treatments dermatologists actually use. Then why the first step is proving it is fungus at all, and the timeline that makes people quit too soon.</p><div class="acv-band"><div class="acv-inner"><div class="acv-big">1</div><div><div class="acv-q">The Treatments</div><h3>Six Treatments Dermatologists Actually Reach For</h3></div></div></div><p>There is no single best option. The <a href="https://www.aad.org/public/diseases/a-z/nail-fungus-treatment" target="_blank" rel="noopener">American Academy of Dermatology</a> says the plan is built around your health, your other medicines, how much nail is affected, and which fungus is there.</p><p>What follows is the working menu, roughly in the order it gets considered.</p><ol class="acv-signlist"><li><b>Prescription solution painted on the nail.</b> Efinaconazole and tavaborole are FDA approved for toenail fungus, applied once a day for about 48 weeks. AAD says this route is often chosen for mild cases and for people 60 and older.</li><li><b>Prescription nail lacquer.</b> Ciclopirox is applied daily and stripped off with rubbing alcohol every seven days. Any loose infected nail has to be removed first for it to work.</li><li><b>Oral terbinafine.</b> A daily pill for 12 weeks to treat toenails. AAD says you need a liver function test before starting and again six weeks in, and no alcohol while you are taking it.</li><li><b>Oral itraconazole.</b> Also daily for 12 weeks for toenails, taken with a full meal. AAD warns it interacts with many drugs including statins, and is not for use in pregnancy.</li><li><b>Softening or thinning the nail.</b> A 40 percent urea cream softens a thick nail so medication can reach further. Debridement, where the dermatologist removes layers of nail, does the same job mechanically.</li><li><b>Removing the nail.</b> Surgical or chemical removal is what is left when everything else has failed. Cleveland Clinic says topical medicines work best when paired with oral ones. It also says oral drugs may offer the most on their own.</p><p>That pairing is why procedures show up in a plan. Filing, thinning and micro-drilling the nail exist to get medicine deeper. AAD says another nail problem can look just like fungus. Nail psoriasis does. So does an old nail injury.</p><p>So the dermatologist takes a sample.
